Payment for services is expected at the time treatment is provided unless other financial arrangements have been made.

As a service to you, our office will submit charges for services to your dental insurance carrier if you have dental insurance coverage.  However, we do consider the patient primarily responsible for the account.  If a problem arises with the insurance, it is between the patient and the insurance carrier. All co-pays and deductibles are due at the time of service.

Although we can submit to all dental insurance plans, we would be considered out of network except for Delta Premier and Anthem Blue 300.  You will need to check your plan to see if and how they benefit out of network.
